Does PT-141 Nasal Spray Work?
Yes, at the right dose. That second part is where things go wrong.
PT-141 (bremelanotide) works in the brain, not the blood vessels. It switches on melanocortin receptors (mainly MC3R and MC4R) that drive desire itself. The nasal route was the original plan. Palatin Technologies developed PT-141 as a nasal spray first. Several human studies tested it, and four tell you what you need to know:
- Men, 2004. In healthy men and men with mild-to-moderate erectile dysfunction, intranasal PT-141 produced erections that beat placebo only at doses greater than 7 mg. The first erection arrived in about 30 minutes. Flushing and nausea were the most common side effects.
- Men, 2005. A low 7.5 mg nasal dose plus 25 mg sildenafil gave a stronger erectile response than sildenafil alone in 19 men.
- Women, 2006. Eighteen premenopausal women with arousal disorder took a single 20 mg nasal dose. More reported moderate or high desire than on placebo (p = 0.0114). Among women who had sex within 24 hours, more were satisfied with their arousal.
- Men who failed sildenafil, 2008. In 342 men, a 10 mg nasal spray gave a positive result in 33.5% versus 8.5% on placebo. One honest flag: the journal attached an expression of concern to this paper in 2023, and the same author's bremelanotide trial in women was retracted. Treat it as weak support.
Notice the doses. 7.5 mg. 10 mg. 20 mg. Not one of the nasal studies that worked used anything close to the 1 or 2 mg that sprays sold today deliver per session.
Why the spray was dropped
Blood pressure ended it. After the nasal trials, the FDA raised concerns about blood-pressure increases, and in 2007 Palatin and its partner King Pharmaceuticals put their phase 3 plans for erectile dysfunction on hold. Later trials had to add intensive blood-pressure monitoring to answer those FDA concerns.
The deeper problem was absorption. A 2017 paper in the Journal of Hypertension explained that the nasal route showed wide variability in bioavailability. Some people absorbed more than they needed and got the side effects. Others absorbed too little to feel anything. Same bottle, very different result.
So the drug was rebuilt as a shot. The subcutaneous injection absorbs at about 100%, which made every dose predictable. That version became Vyleesi, approved on June 21, 2019 for premenopausal women with acquired, generalized low sexual desire (HSDD). There is no approved PT-141 nasal spray. Every spray on the market is either compounded under a prescription or sold by a peptide vendor.
PT-141 Nasal Spray Dosage: The Per-Spray Math
Here is the number nobody quotes. It sits in the pharmacology section of the Vyleesi prescribing label, inside a study that gave people 20 mg of nasal bremelanotide:
From the FDA label: the 20 mg intranasal dose reaches a mean peak blood level 2.5 times higher than the 1.75 mg Vyleesi injection.
Run the math. 20 mg is 11.4 times the injected dose, yet it produces only 2.5 times the peak. Per milligram, the nose delivers roughly 22% of the peak the shot does. If absorption scales in a straight line (an assumption, since nobody has published a full dose curve), you would need about 8 mg in the nose to match one 1.75 mg injection.
Two independent data points agree. The label math says about 8 mg. The 2004 men's trial found the effect started above 7 mg. That is the realistic nasal target, not 1 to 2 mg.
What bottles actually contain
Now compare that with what you can buy. Two real examples from the current market:
- A vendor spray: 40 mg per bottle, 80 sprays, 500 mcg per spray, in saline with a benzalkonium chloride preservative. Priced at $110.
- A telehealth compounded spray: 5 mg/mL in a 5 mL bottle, 0.1 mL per spray, so 0.5 mg per spray. Directions start at 1 mg (two sprays) and cap at 2 mg per session. Priced at $199.
At 1 to 2 mg nasal, the label math puts your peak level near what roughly 0.2 to 0.45 mg injected would give. In the dose-finding trial for Vyleesi, even 0.75 mg injected did not differ from placebo; 1.25 and 1.75 mg did. That is the most likely reason a spray feels like nothing.
How to read any PT-141 spray label
- Dose per spray = concentration (mg/mL) × spray volume (mL). Most pumps deliver 0.1 mL.
- Example: 5 mg/mL × 0.1 mL = 0.5 mg per spray.
- Sprays per bottle = bottle volume ÷ spray volume, minus the priming sprays.
- If a label lists only "mg per bottle", divide by the stated number of sprays. No spray count, no way to dose it.
Should you just use more sprays? Not on your own. The per-occasion limit on Vyleesi exists because of blood pressure, and the nasal route's whole history is unpredictable absorption. Nasal Spray vs Injection vs Pills vs Sublingual
Four forms, one molecule. Only one has solid numbers behind it.

The spray's one real edge is speed. Nasal PT-141 peaked in about 30 minutes in the 2004 trial, against about an hour for the injection. If spontaneity matters to you, that half hour is a genuine difference. The trade is a shorter half-life and far less predictable dosing. PT-141 pills, oral and sublingual: do they work?
Probably not in any useful amount. Here is why.
PT-141 is a seven-amino-acid peptide. The Vyleesi label notes that its main breakdown route is hydrolysis, the same chemistry your gut uses to digest protein. When researchers gave it to dogs by mouth, they found minimal oral absorption. No human study of an oral PT-141 pill has been published.
For scale, look at the one peptide pill that made it to market. Oral semaglutide is built with an absorption enhancer called SNAC, and even so, the label puts its absolute bioavailability at about 0.4% to 1%. A PT-141 capsule with no enhancer has no reason to do better.
Sublingual troches have a slightly better case, because the lining of the mouth skips the stomach. But the best result so far came from an engineered suction patch stuck to the inside of the cheek, which reached 26% of the injection's exposure in dogs. A troche that dissolves under your tongue is not that device. Nobody has measured what it delivers.
A pill that "works" deserves a second look. If an oral PT-141 product produces a strong effect, ask what else is in it. Absorption this low leaves little room for the peptide to be doing the work.
Side Effects of PT-141 Nasal Spray
Same molecule, same risks. The route does not erase them.
The injection's label gives the clearest picture, from the phase 3 trials:
- Nausea: 40% of users. 13% needed anti-nausea medicine and 8% quit the trials over it. For most people it eased by the second dose.
- Flushing: 20.3%. Headache: 11.3%. Vomiting: 4.8%.
- Blood pressure: a temporary rise of up to 6 mmHg systolic and 3 mmHg diastolic, peaking 2 to 4 hours after the dose, with the heart rate dropping by up to 5 beats per minute.
- Skin darkening: focal hyperpigmentation of the face, gums or breasts in 1%, more likely with daily use and darker skin. It did not always fade.
- Other medicines: PT-141 slows stomach emptying and can sharply cut the absorption of oral naltrexone.
Nasal use adds its own layer: in the 2004 nasal trial, flushing and nausea again led the list. And because nasal absorption varies so much, the dose that gives one person nothing can give the next person a pounding headache. PT-141 Nasal Spray Reviews and Reddit: How to Read Them
Reviews tend to fall into two camps. It worked, or it did nothing.
Both can be telling the truth. With dosing this variable, a wide spread of results is exactly what the 2017 blood-pressure paper described. Before you trust any review, check four things:
- The dose per session. A "did nothing" report at 1 mg tells you about 1 mg, not about PT-141.
- The timing. Nasal PT-141 peaks fast and fades faster than the shot. Spraying three hours early can miss the window.
- Side effects. Mild flushing or nausea is not proof of absorption, but a spray that never produces any at a decent dose is a clue.
- The source. A spray from a prescribing pharmacy and a spray from an anonymous vendor are different products, even with the same name on them.

Where to Buy PT-141 Nasal Spray
Three routes exist. They differ more than the price tags suggest.
1. A prescription compounded spray. This is the legal path for a PT-141 nasal spray in the US. A telehealth or men's-health clinic assesses you, writes the prescription off-label, and a compounding pharmacy makes the bottle. You get a known concentration, a real label and someone who can check your blood pressure. The catch is price: the example above was $199 for 25 mg of peptide.
2. Vyleesi. If you are a premenopausal woman with HSDD, this is the FDA-approved version, and your doctor can prescribe it. It is an injection, not a spray. There is no nasal Vyleesi.
3. A vendor spray or vendor vial. Peptide vendors sell ready-made sprays and freeze-dried vials. Sprays are convenient but hide the dose problem inside the bottle. A vial gives you control of the math, and if you are comparing PT-141 for sale across vendors, a 10 mg vial is the standard size and the easiest to check against a batch certificate.
