Enclomiphene Dosage Chart & Bloodwork Guide.
Enclomiphene is a selective estrogen receptor modulator (SERM) that blocks pituitary estrogen receptors, increasing LH and FSH to stimulate natural testosterone production while preserving fertility and sperm production.
12.5 mg once daily, oral, in the morning. Recheck a morning panel (total/free testosterone, LH, FSH, estradiol, hematocrit) at 6-8 weeks, and move to 25 mg only if levels or symptoms are still short.
Dosage Protocols
Doses are whole compounded capsules taken at the same hour each morning: no vial, no syringe maths. Only 12.5 mg and 25 mg were ever carried through phase III.
The lower of the two doses carried through phase III, and where almost everyone should start.
Why this dose+
In the phase II ZA-203 trial, 12.5 mg took a baseline of 217 ng/dL to 472 ng/dL at 3 months, while 25 mg went 210 → 406 ng/dL. Starting here rather than at 25 mg is what makes the 6-8 week lab draw informative: you find out whether you needed more.
Titrate here only if the 6-8 week panel leaves testosterone under 400-450 ng/dL or symptoms have not moved.
Why this dose+
In the 6-week phase II parallel-group trial this dose averaged 604 ± 160 ng/dL, statistically indistinguishable from testosterone gel at 500 ± 278 ng/dL (p=0.23). In a separate phase IIb trial, three months at 25 mg raised LH from 5.3 to 11.9 and FSH from 9.4 to 14.9 mIU/mL.
Some prescribers drop to alternate-day dosing once levels normalise; no trial tested this schedule.
Why this dose+
The rationale is that testosterone stayed elevated for at least 7 days after the last dose in trial subjects. It is clinical improvisation rather than evidence, so the change needs its own confirmatory lab draw 6-8 weeks later.
- Clomiphene citrate is a mixture: roughly 62% enclomiphene, 38% zuclomiphene.
- A 50 mg Clomid tablet therefore delivers about 31 mg of enclomiphene; 25 mg delivers roughly 15.5 mg.
- The missing isomer is the difference, not the dose: zuclomiphene runs ~30 days, enclomiphene ~10 hours.
- So switching between the two drugs is an isomer swap, not a milligram conversion.
- Enclomiphene lifts the estrogen feedback the pituitary sees, which is what raises LH and FSH.
- Once that loop is fully released, extra milligrams have nothing left to unblock.
- The review literature describes steady-state levels as maximised at 25 mg/day.

Weekly Schedule
Both studied doses were taken every single morning, with no days off. The alternate-day row is a maintenance improvisation; no trial ever tested it.
Missed a dose? Take it the same day if you remember; otherwise skip it and take your normal capsule, rather than doubling up. Trial subjects kept elevated testosterone for at least 7 days after their last capsule, so one skipped day does not undo anything. Keep the timing consistent so your 6-8 week lab draw reflects your steady state.
What to Expect
Gonadotropins move in the first week, testosterone lands in range inside six, and the plateau arrives by month three. No trial ever measured whether you feel different.
Peak blood levels arrive 2-3 hours after the capsule, and pituitary estrogen receptors are blocked from day one. LH and FSH rise first as the negative feedback lifts, and testosterone follows them. Testosterone gel does the opposite and suppresses both. Nothing is noticeable this week.
Fourteen days of dosing puts most men in range, and by week 6 the 25 mg arm averaged 604 ± 160 ng/dL, statistically indistinguishable from transdermal testosterone at 500 ± 278 ng/dL. This window is where the 6-8 week lab draw belongs, and where the 12.5 mg → 25 mg titration decision gets made.
In the phase II ZA-203 trial, 12.5 mg took a baseline of 217 ng/dL to 472 ng/dL by month 3 and 25 mg took 210 ng/dL to 406 ng/dL, then levels held. No enclomiphene trial measured symptoms or quality of life with a validated instrument. The FDA’s 2015 Complete Response Letter and the EMA’s 2018 refusal both cited that gap.
Over 16 weeks of phase III, sperm density rose 11.7-15.2% on enclomiphene while the testosterone gel comparator dropped it 32.8-56.6%. In the 6-month phase IIb study, 6 of 6 men on enclomiphene had elevated counts (75-334 million/mL) versus 2 of 5 on gel. In the 12-month obesity trial every arm, including placebo, followed a structured diet and trained with a personal trainer at least three times weekly.
You are prompting an axis, not replacing one. Stopping does not create the shutdown-and-recovery problem that coming off TRT does; there is nothing suppressed to restart. Testosterone stayed elevated for at least a week after the last dose in trial subjects, then drifts back. Nearly every trial participant was an overweight man with obesity-linked secondary hypogonadism, and the 12-month phase II study paired the drug with diet and supervised training.

- No pooled incidence rates have ever been published for enclomiphene.

- Breast tenderness or swelling.
- Hematocrit climbing toward 54%.
- Testosterone still under 400 ng/dL at the 8-week draw.
- Any visual change that persists: clomiphene’s FDA label warns scintillating scotomata may be prolonged or irreversible, worsening with higher dose or longer duration (~1.5% incidence).
- Calf swelling, pain or warmth.
- Sudden breathlessness or chest pain: the EMA refused approval over venous thromboembolism risk, raised as a qualitative concern with no published incidence figures for enclomiphene.
- One-sided weakness, facial droop or slurred speech.
- Yellowing of the skin or eyes.
- Primary (testicular) hypogonadism (Klinefelter, post-chemotherapy or radiation, orchiectomy), because a louder pituitary signal has nothing to act on.
- Any liver disease or abnormal LFTs, contraindicated on the clomiphene label.
- Personal or family history of DVT, pulmonary embolism or a known clotting disorder, given the EMA’s venous-thromboembolism refusal ground.
- Known hypersensitivity to clomiphene/Clomid, an explicit trial exclusion.
- Untreated pituitary tumour or other intracranial lesion.
- Uncontrolled thyroid or adrenal dysfunction.
- Hematocrit above 54%, a trial exclusion.
- Active or prior prostate or breast cancer.
- Concurrent or recent exogenous testosterone: trials excluded any T use within 6 months.
- Women who are pregnant or may become pregnant: clomiphene is pregnancy-contraindicated with reported fetal anomalies.
- Anyone under 18.
- Self-directed use without a baseline LH/FSH panel, the only labs that show whether the drug can work at all.
FAQ
Is enclomiphene FDA-approved?+
No, and it is important to know why. Repros Therapeutics developed it as Androxal, filed an NDA and received a Complete Response Letter in December 2015, in which the FDA stated the phase III study design was no longer adequate to demonstrate clinical benefit; the trials produced a testosterone number, not evidence that men felt or functioned better. The advisory committee meeting scheduled for November 2015 was cancelled and never held. The EMA refused marketing authorisation for the identical molecule (EnCyzix) in January 2018, citing both the missing symptom evidence and venous thromboembolism risk, and development was formally discontinued in April 2021. In the US it exists only as a compounded prescription product. It has never been added to the FDA’s 503A bulk drug substances list, which is what makes telehealth supply legally unsettled rather than routine, and it means there is no manufacturer label, no standardised strength, and no guaranteed potency behind whatever capsule you are handed.
Enclomiphene vs clomiphene: is it just the cleaner half?+
Broadly yes, and the advantage is real but smaller than the marketing implies. In a 66-man retrospective where patients took both sequentially, enclomiphene raised testosterone a median 166 ng/dL versus 98 ng/dL for clomiphene, a difference that did not reach statistical significance (P=0.20). Where it clearly separated was tolerability: estradiol moved −5.92 pg/mL versus +17.50 pg/mL (P=0.001), with significantly less decreased libido (P=0.001), reduced energy (P=0.044) and mood change (P=0.03), and about five-fold lower odds of any documented adverse event (OR 0.18, 95% CI 0.07-0.44). Set against that, clomiphene is FDA-approved (in women), generic, cheap and pharmacy-standardised. Enclomiphene is none of those.
Will it shrink my testicles or damage fertility the way TRT does?+
No. It does the opposite, and this is the single most robust finding in the whole evidence base. Over 16 weeks of phase III, enclomiphene held sperm concentration in the normal range with density up 11.7-15.2%, while the testosterone gel comparator cut sperm counts 32.8-56.6%. In the 6-month phase IIb study, 6 of 6 enclomiphene men had elevated counts (75-334 million/mL) against 2 of 5 on gel. The AUA testosterone deficiency guideline reflects this split directly: SERMs are a conditional option (Statement 27, Grade C) for men wanting to preserve fertility, while exogenous testosterone carries a strong Grade A recommendation against use in men currently trying to conceive.
What happens when I stop taking it?+
There is no suppression to recover from, because you were never replacing testosterone; you were prompting your own pituitary. Trial subjects kept elevated testosterone for at least 7 days after the final capsule despite the ~10-hour half-life, then it drifts back toward wherever their own axis naturally sits. Being honest about the gap: no trial followed men long enough after discontinuation to say how fast or how completely that reversal happens, so anyone quoting you a precise figure is extrapolating. If the underlying driver was body fat, poor sleep or overtraining, that is what determines your landing point.
Should I be on 12.5 mg or 25 mg?+
Both went through phase III and both work; the question is how much headroom you actually need. Convention is to start at 12.5 mg daily and titrate to 25 mg only if the 6-8 week morning panel leaves total testosterone short of roughly 400-450 ng/dL or symptoms have not shifted. Opening at 25 mg is not dangerous, but it destroys the information; you will never know whether you needed it, and estradiol effects plus side effects track dose. Going above 25 mg is off the map entirely: that is the top of the trial evidence, and steady-state levels stop climbing meaningfully past it.
Does it work if my low testosterone is from age rather than weight?+
The evidence is thin and you should treat it that way. Nearly every trial enrolled overweight or obese men aged 18-60 with confirmed secondary hypogonadism: two separate morning testosterone readings at or below 300 ng/dL, with LH still in a responsive range (1.4-9.4 mIU/mL in the 12-month study), BMI 30-42, waist ≥40 inches. That is a specific patient. The mechanism requires a pituitary that will respond and testes that can answer; both become less reliable with age, and no trial specifically tested men over 60. If your LH is already high, your problem is testicular and this drug cannot fix it.
Do I need anastrozole or another AI alongside it?+
Usually not, and adding one reflexively is a common self-inflicted injury. Enclomiphene already antagonises estrogen receptors at the pituitary, and in head-to-head data its net estradiol change was slightly negative rather than positive. Estradiol is not a waste product in men; it drives bone density, libido and lipid handling. Stacking an aromatase inhibitor on top of a SERM is one of the more reliable routes to flat libido, aching joints and low mood, which people then blame on the enclomiphene. If estradiol genuinely runs high on labs alongside symptoms, that is a dose conversation with a prescriber, not a second drug you add blind.
Which labs do I need, and when?+
Baseline: two separate early-morning total testosterone draws (the AUA requires two before diagnosing deficiency), plus free testosterone, LH and FSH (those two are what distinguish secondary from primary hypogonadism and determine whether the drug can work at all), estradiol, SHBG, prolactin, a CBC for hematocrit, LFTs, a lipid panel, and PSA if you are over 40. Recheck at 6-8 weeks to make the titration call, then every 6-12 months per guideline. Watch hematocrit even though you are not taking exogenous testosterone: the trials excluded men above 54% and that remains the practical line.
